Though rare, fatal accidents on movie sets are, sadly, nothing new. Decades of lax safety concerns stemming from the early days of Hollywood have led to an unfortunate number of injuries and fatal accidents over the years. While there have been recent advances in set safety and overall precautions, that still hasn't managed to eliminate the possibility of something going horribly wrong on a studio set. Some of these deaths were horrible accidents that no one could have predicted while others are the result of shocking negligence and a desire to cut costs.
